Please see below for a message from Wellington-Dufferin-Guelph Public Health.
Immunization notices have been sent for elementary students who do not have up-to-date immunization records. Please check your mail.
What to do if you received a notice:
If you received an immunization notice it means that our files show that your child has an incomplete immunization record or has not had all their vaccines. It is a parent/guardian’s responsibility to report their child’s vaccinations to Public Health. Healthcare providers are not required to do this.
The deadline to have an up-to-date immunization record with Public Health is March 14, 2025, to avoid possible suspension on March 19, 2025.
Updating your records is easy:
· Visit immunizewdg.ca
· Email vaccine.records@wdgpublichealth.ca
· Call 1-800-265-7293 ext. 7006
If your child needs a vaccine:
Make an appointment with their healthcare provider and bring the notice with you to the appointment, then report the vaccinations to Public Health or book an appointment with Public Health at wdgpublichealth.ca/appointments or call 1-800-265-7293 ext. 7006.
