• Community Hours: 40 Hours required 
  Community Hours Form

• E-learning: 2 E-learning courses required 
  OR complete the Opt-Out form
  Online Learning Opt Out Form

Both Forms need to be submitted to Student Services to earn your requirements to graduate.
